Cindy was a thirty-seven-year-old bank executive who realized that she had a drinking problem. As an illustration, within the past two months she has felt the need to have a drink or two before going to work, a week ago she failed to pass a random hair alcohol test at work, three weeks ago she got arrested for a DUI, and lastly, for about two months she has started to forget what she says and does when she drinks.
Not unlike other people, Cindy's alcohol involvement started out slowly and continued at this level of involvement for quite a long time due to the fact that every now and then she engaged in casual social drinking. In actual fact, for approximately a year, every time she drank, she made sure to drink in moderation. Something about her drinking, nevertheless, seemed to radically change when she divorced her husband.
Cindy got extremely disheartened about the breakup with her husband, and as a way to stop dwelling on her negative feelings she made up her mind that she would start palling around more frequently with some of her pals who love to drink. It didn't take long, however, before her drinking increased considerably the more frequently she went to and drank at happy hours, private parties, family get-togethers, dinner dates, and sporting events with her pals.
In short, she was drinking and having a great time just like everyone else in her group of friends without giving much thought to the alcohol short term effects and especially about the alcohol long term effects she has been going though. Yet somewhere in her mind she knew about the adverse alcohol effects on the body and on the brain and that she probably needed alcohol treatment but avoided the thought as much as possible.
One morning during her yearly physical, her healthcare practitioner asked her if she drank alcohol. Not wanting to lie to her physician, Cindy acknowledged that she often drinks more than she should. In fact, she stated that she regularly drinks in an excessive and irresponsible manner. Then Cindy told her healthcare practitioner about her depression. More specifically, she mentioned that broken relationships many times triggered a disheartening sequence of events characterized by increased drinking which further led to more disheartening feelings that, in turn, led to more drinking. And this is exactly what happened when she and her husband got divorced two years ago.
Cindy's physician then stated the following: "I am not trying to make a snap diagnosis, but with your medical situation we may be facing two separate issues. As a result, I think we ought to make an appointment for you to get an alcohol abuse and alcohol dependency assessment from my partner, Dr. Johnson, who is a chemical dependency and substance abuse specialist."
"Whether your drinking circumstance is more associated with alcohol abuse or alcohol addiction is unclear, but I think that further exploration is reasonable. Then I believe we ought to schedule an appointment for you to get a psychological exam from another one of my partners, Dr. Devor, who is a psychologist."
"I want to get a better handle on your depression and see how much your drinking and depression are related." Cindy expressed her agreement with her doctor's strategy and thanked him for his assistance.
